The Children’s Team at Excellus is looking to collaborate and engage with community agencies who provide support to homeless youth in our area. We would like to invite anyone in the group who may be working with a youth who is dealing with homelessness and has Excellus Medicaid to contact me for any case management collaboration that may be helpful.
