Coordinated Entry

Who We Are

Coordinated Entry is the primary source for referrals for HUD funded housing programs in Monroe County. It’s a collaborative effort between Partners Ending Homelessness and Coordinated Care Services, Inc.

  • Standardized tools and practices – Consistent process to identify appropriate housing interventions.
  • Streamlined access and referral – Fair and equitable.
  • Continual improvement – Data-informed decision-making.
  • Community-wide prioritization – Identify housing for persons with the highest needs.

Our Goals

  • Decrease the length of time persons experience homelessness.
  • Reduce the rate of returns to homelessness.
  • Increase successful placements into and retention of permanent housing.

What We Offer

  • A connection between supportive services and those who are experiencing homelessness or at-risk of homelessness.
  • A collaboration of homeless service providers with other essential community stakeholders to create the culture needed to help end homelessness.

The Entry Process

The coordinated entry process has been streamlined to help identify permanent housing for those experiencing homelessness as efficiently as possible. For more details, please visit the CE Resources and Prioritization List page.

305 Households Housed

In 2025, we housed 305 households in a permanent housing solution. On average, the housing process took 20 days.


CCSI is responsible for facilitating the development and implementation of the Coordinated Entry system in Monroe County. Coordinated Entry is funded by the Emergency Solutions Grant Program through the City of Rochester Department of Neighborhood and Business Development, the Monroe County Department of Community Development, and by the US Department of Housing and Urban Development Continuum of Care Program.
