This is OMH yearly outreach to get feedback about how services are going for people in the mental health, substance use, and/or intellectual and developmental disability systems in Monroe County to help with planning for future services. See LINK below!

• People who have experience getting services in the mental health, substance use, and/or intellectual and developmental disability systems and their family members
• Clinicians, case workers, case managers, direct care workers, etc. who have direct contact with people who get services to pass along the survey for consumers/family members to complete or assist with completion.
